چکیده

We present the outcome of non-operative management of fractures of the shaft of the femur in children. Thirty children with non-operatively managed unilateral fractures of the shaft of the femur were studied. The mean age was 6.5 years (range: 1.2 to 12). Axial, angular and rotational malunion were assessed clinically and radiologically by plain films and computed tomography (CT). All children had shortening of the fractured limb with a mean shortening of 1.8 cm (range: 0.5 to 3.5). A significant level of shortening was seen in children over 7 years. In the sagittal plane, all had anterior angulation ranging from 4 degrees to 31 degrees (mean: 17.8 degrees) and in the coronal plane, lateral angulation (varus) was seen in 90% (27 patients) with a mean angle of 8.9 degrees. Rotational malunion ranged from -43 degrees to +43 degrees (mean: 9.5 degrees), 30% had rotational malunion of > 20 degrees compared to the uninjured side; 73% had internal rotation malunion and 27% had external rotation malunion. We conclude that non-operative management of fractures of the shaft of the femur in children often results in malunion at the fracture, particularly in the older age group and in comminuted fractures. Perhaps a longer follow-up might have shown lower levels of malunion. Frequent monitoring of fracture position may reduce the occurrence of deformity. It may be appropriate to consider reduction and operative stabilisation of such fractures whenever neces-

Elastic Stable Intramedullary Nailing of Femoral Shaft Fracture-Experience in 48 Children

Background: Femoral shaft fractures are an incapacitating pediatric injury accounting for 1.6% of all pediatric bonyinjuries. Management of these fractures is largely directed by age, fracture pattern, associated injuries, built of the childand socioeconomic status of the family. We retrospectively evaluated the use of elastic stable intramedullary nail (ESIN)in surgical management of femoral s...

The Role of Biplanar Distal Locking in Intramedullary Nailing of Tibial Shaft Fractures

Background: To compare the union times of the uncomplicated tibial shaft fractures, which were distally locked by twocoronal and one sagittal screws and by only two coronal screws. Methods: 45 patients with tibial shaft treated with intramedullary nailing included in this study. 23 of 45 fractureswere treated with uniplanar two distal interlocking (Group 1) and 22 fractures were treated with bip...

Outcomes of the isolated closed tibial shaft fractures treated nonsurgically

Abstract   Background: Fractures of the tibia are important for their commonness and controversy in their management. Both conservative and surgical techniques have been introduced in an effort to speed time to :::union::: while minimizing the occurrence of complications. Standard treatment for low-energy tibial shaft fractures includes closed reduction and cast immobilization. The purpose of...

Injuries Associated with Femoral Shaft Fractures with Special Emphasis on Occult Injuries

Background: Fractures of the femoral shaft are mostly the result of high-energy accidents that also cause multiple trauma injuries, in particular ipsilateral knee and hip injuries. The purpose of this study was to investigate the incidence of injuries associated with femoral shaft fractures and how many of them were undetected. Methods: We studied 148 patients (150 femoral shaft fractures) wi...

Femoral fractures in infants: a comparison of Bryant traction and modified Pavlik harness.

Bryant traction is a commonly used method for femoral shaft fractures in children, but many disadvantages have been reported. Pavlik harness with exact clinical effect and fewer complications has gained increasing popularity in resent years. The objective of this study was to evaluate and compare modified Pavlik harness with Bryant traction for infant with a femoral shaft fracture. A retrospect...
